When you partner with Verity, you can choose to either have Verity’s team of care navigators assist with routing patients into care, or have the providers book through Verity.
Once you’re all set up, you send clients our way and we schedule them with mental health professionals that match their requested criteria. After each session, we’ll send clients brief feedback forms so we can take preemptive action for clients at risk of dropping out of treatment. Because we also consolidate and track client’s health history in one centralized location, we can provide predictive analytics to help connect you with clients who may benefit from mental health resources.
Research shows that 60% of discharged clients don’t go into outpatient care. Although the time when patients transition from different levels of care is one when they are most susceptible to risk, most healthcare systems don’t have good ways of connecting patients with providers that have immediate availability, ways of confirming their patients are attending their sessions, and ways of tracking their progress as they go through their treatment program.
Poor follow-up means clients being lost to care or clients that terminate with poorly-matched providers prematurely, ultimately contributing to high readmission rates. This negatively influences your HEDIS scores and reimbursement rates and opens you up to clawbacks.
